Legal
As a psychiatric practice, we handle protected health information under HIPAA. This is a summary; you'll receive the full notice, and be asked to acknowledge it, before your first appointment.
To provide, coordinate, and bill for your care; to consult with other clinicians involved in your treatment, with your permission; to meet legal, safety, and public health obligations; and to run the practice (scheduling, quality review, staff training).
We won't share your information with family members, schools, employers, or other providers for purposes outside your care. Marketing and most research uses also require your authorization.
You may request a copy of your records, ask us to correct them, request restrictions on certain uses, ask for confidential communications (such as a specific phone number or email), receive an accounting of disclosures, and receive a paper copy of this notice.
There are narrow situations where we must share information without your permission — for example, suspected abuse or neglect, a court order, or an imminent risk of serious harm to you or someone else.
If you believe your privacy rights have been violated, tell us at help@hwpsych.com or contact the U.S. Department of Health and Human Services Office for Civil Rights. We will never retaliate against you for filing a complaint.
