This policy explains what happens to information you share through this website. It covers the website only — health information shared as a patient is governed by our Notice of Privacy Practices.
When you send the appointment request form, we collect your name, email address, phone number, who the care is for, your visit and contact preferences, and anything you write in the message field. We also receive basic, aggregated information about how the site is used.
We use your details to reply to you, to schedule and prepare for an appointment, and to keep a record of your request. We do not sell your information, and we do not use it for advertising.
Requests are visible only to the Haven Wellness Psychiatry team. We use trusted service providers for hosting and scheduling, and they may process data on our behalf under confidentiality obligations.
The website form is not a secure clinical channel. Share general information only; detailed history belongs in your appointment or a patient portal.
You can ask us to correct or delete the information you submitted through this website at any time by emailing help@hwpsych.com. We'll respond within a reasonable period, subject to record-keeping requirements.
